Who Is Eligible To Join?

• Independent Physicians who are not employed by a competing delivery system
• Bellin Medical Group Employed Physicians
• Mid-level Practitioner affiliated with an independent or Bellin employed physician

Bellin Hospital is currently the sole hospital member.

Why Should I Join?:

In addition to focusing on improving care, reducing cost, and competing for marketshare, PPL has put together opportunities to help you reduce the cost of running a practice. We want you to assist you in being a viable, sustainable option within the delivery system and the consumer marketplace.

Governance has been established to optimize physician involvement and decision-making in partnership with the Hospital member. The governance structure is balanced and every member has equal and fair representation through their elected Board of Directors representatives.

How Do I Join?:

Complete:  Application to Request Membership to PPL Provider Network

• Annual individual membership fee as determined by Board of Directors
• Sign individual Membership Agreement
• Sign individual Participating Physician Agreement
• Two elected Physician Board representatives for each physician membership class
• Invest individual time and energy into organizational initiatives
